You should know this about us-The System's 947-bed, tertiary medical center, University Hospital,
is the primary affiliate of Case Western Reserve University (CWRU). Together, they form the largest
center for biomedical research in the State of Ohio. The System provides the major clinical base
for translational researchers at the Case Research Institute, a partnership between UHC and CWRU
School of Medicine, as well as a broad and well-characterized patient population for clinical trials
involving the most advanced treatments.

So,  you've made an appointment with us, and you're a little nervous about what's going to happen-

Let's go over just that-

If you have never seen one of our specialists before, you will be asked to fill out a history
BEFORE you come to your appointment.  Doing this allows you to look up things that you might not remember, or to check with other people to get information. 

Doing this before your appointment also allows us to spend more time with YOU, instead of with your forms; as would be the case if we asked you to do this during your appointment.

  • Fill out the form to the best of your ability and knowledge.

  • If you have questions about the form, call 216-844-6940 and we'll help you.

  • Contact any physicians who have treated you in the past, and get all related tests and results. 
    If possible, we would like the ORIGINAL EEG print outs, MRI and PET scan FILMS as well as the reports.

  • Bring your completed form, your records, films and old results to your appointment.
