So, you've made an
appointment with us, and you're a little nervous about what's going to happen-
Let's go over just that-
If you have never seen one of our
specialists before, you will be asked to fill out a history
BEFORE you come to your appointment. Doing this allows you to look up things that
you might not remember, or to check with other people to get information.
Doing this before your appointment
also allows us to spend more time with YOU, instead of with your forms; as would be the
case if we asked you to do this during your appointment.
If you haven't received one yet,
and you'd like to get started you can DOWNLOAD and PRINT this form by
clicking HERE.
(hint-
If you do decide to do this, call 216-844-6940 and tell us, so that you won't get the same
thing in the mail.)
Fill out the form to the best of your
ability and knowledge.
If you have questions about the form,
call 216-844-6940 and we'll help you.
Contact any physicians who have
treated you in the past, and get all related tests and results.
If possible, we would like the ORIGINAL EEG print outs,
MRI and PET scan FILMS as well as the reports.
Bring your completed form, your
records, films and old results to your appointment.
